All board members, employees, contractors, and volunteers of the School are also required to respect and maintain the confidentiality of all information, including but not limited to, student enrollment and/or performance records, documents, reports, files, correspondence and communications, to which the employee or volunteer has access in carrying out responsibilities and duties both during and after employment or service with the School.

All board members, employees, contractors, and volunteers are expected to show the highest regard for the privacy of each student and the School and will strictly observe the confidentiality of records and other information associated with students and the School. Confidentiality is essential to the sound relationship with our students; it is also a legal and ethical matter of the utmost importance. Any discussion of confidential information outside the property or similar violation of these standards may result in disciplinary action up to and including termination for employees and suspension of service for volunteers.
Consent to Use of Electronic Signature
I have read, understand and agree to the terms and conditions of the Disclosure and Consent to Use of Electronic Signature. I hereby consent to the use by the School of my electronic signature, if provided. I understand that my electronic signature is legally binding and that my electronic signature may not be invalidated solely on the basis that the signature was electronically obtained. I understand that I may decline to consent to the use of an electronic signature by not signing this form below. However, I understand that if I decline, I must complete all forms/contracts on paper and return a handwritten signed copy to the School by the respective deadlines.

This will acknowledge my receipt of Isidore Newman School Confidentiality Policy and my responsibility to become familiar with its expectations and course of action should I be privy to confidential information during the term of my employment or service. I also acknowledge that I have read the Disclosure Regarding Electronic Signature below.
(Employment, Contract, or Volunteer)
Disclosure Regarding Electronic Signature
Isidore Newman School, (the “School”) is implementing an Electronic Signature Program. Under this system parents and legal guardians will have the option to sign School documents electronically. Please carefully read the following information. To consent to the use of an electronic signature, please sign this form and return it to the School.

1. You may choose not to consent to the use of an electronic signature. However, the School strongly urges you to consider the benefits of an electronic signature, including better data accuracy, a more green approach to paper management and less of a burden for the School's administration.

2. By your signature below, you agree and consent to receive electronic records of the terms of any forms, documents, and contracts that the School elects to provide electronically (including, but not limited to, application/admission documents, enrollment/re-enrollment contracts, acknowledgment forms, health forms, and participation and release forms), in lieu of a hard or paper copy thereof. Specifically, you consent to the use of electronic documents, email delivery of documents, and electronic signatures in any communication or agreement involving you, your child(ren), and the School.

3. You have the right to withdraw your consent to having records provided to you in electronic form. If you wish to withdraw such consent, please contact the School at security@newmanschool.org. If you withdraw your consent to receive records in electronic form, the School will mail you paper copies of the records, if any, that the School is required to make after the withdrawal of your consent; however, the withdrawal of your consent will not affect the legal validity or enforceability of any document that you have previously signed electronically.

4. You have the right to obtain a paper copy of an electronic record. If you wish to request a paper copy, please contact the School at (504) 899-5641.

5. To help ensure the authenticity and security of documents with an electronic signature, you will also receive an email confirmation when you sign a document electronically. To effectively participate in the Electronic Signature Program, you are required to inform the School immediately of any changes in your email address.

6. In order to access and retain the electronic records, you must have: a computer or other device capable of accessing the Internet, an Internet Web Browser, an active email account, and a PDF reader capable of viewing and printing the document, such as Adobe Reader. You hereby agree that you are able to access and retain the records in this format and you are satisfied with the necessary software and hardware requirements.

7. You agree and consent that your use of a key pad, mouse or other device to select an item, button, icon, checkbox, to enter text, or to perform a similar act/action, (e.g., clicking “Submit”) constitutes your electronic signature and signifies your intent to be bound. You understand that your electronic signature is legally binding, just as if you manually signed a paper document in ink.
